During your participation in your employer’s AssetMark 401(k)
Plan, you may need to complete forms. For example, if you wish
to change your beneficiary designation or change your salary
deferral amount, you will need to complete a form. The forms you’ll
need are available here, online, so you can access them anytime
you need them.
To use this feature, you will need Adobe Acrobat. If you do not
currently have Adobe Acrobat, click
here for a free download of the software. Forms are provided
in a .pdf format. To access the forms:

Review the list of forms below. After
you’ve found the one you need, click on the title of the
form. It will open in .pdf format through Adobe Acrobat.

You can complete the form online, or print it
to complete later.
•
After you’ve completed the form online,
print the form and sign it if applicable.
•
Give the printed, signed form to your Plan
Administrator for processing.
